Process without bureaucracy: keeping a smaller set of rules

Every process should be traceable to a failure it prevents. When a team can't name that failure, the process is probably inherited rather than needed.

We keep a short standing list of the failures our process exists to prevent, reviewed twice a year, and retire any rule that no longer maps to one. It keeps the rulebook thin enough that people actually read it.
